Terms & Conditions: The rebate is available in US only. Eye exam within the last 90 days. Limited during promotion period to one (1) offer per person. Group or organization requests are void. Rebates coupon will come with your shipment and can only be sent to street or rural addresses. Requests from P.O. boxes will not be honored. Original mail-in certificate(will come with your shipment), original sales receipt and a copy of valid eye exam receipt are required. Purchase must be made between Sept 1, 2010, within 90 days of eye exam, to December 31, 2011. Please allow 6-8 weeks for processing. Offer good only in the USA and Puerto Rico.
